Advice I got from my Federal representative’s office: gather all the information regarding your complaint and start a case with your state’s insurance board.

Fille that appeal and if it still gets denied then file complaint/review with the Insurance Board who will then review. This is what we had to do over life saving skin grafting that was denied to be reversed and ordered it was to be covered.
