the simplest way you can explain it is "too much cyberwear + mental illness/bad mental state = cyberpsychosis. this is why David starts deteriorating faster the worse things get, and why David had such a high tolerance to cyberwear in the first place, he grew up with a loving mother, was enrolled in a top arasaka school, had a sense of community in maines gang. this is also why someone who's gone psycho can be pulled back to sanity by someone they love or care about, like maine being able to recognize David, and Lucy pulling David back.
