Knee Replacement - Surgical Approach . The medial parapatellar approach is a widely used surgical technique for #kneearthroplasty and has been the gold standard since von Langenbeck first described (1879). The approach provides excellent, extensile exposure to the knee joint. It begins with an anterior midline skin incision performed with the knee in flexion. The arthrotomy follows the medial border of the quadriceps tendon, leaving a small tissue cuff on the vastus medialis for later repair. It then extends around the medial side of the patella and continues along the medial border of the patellar tendon onto the anteromedial tibia. The patella is then everted or subluxated laterally and the knee is flexed to 90 degrees, providing full visualization of the joint surfaces for component placement. . The primary advantage is the reliability + extensility of this exposure. It adapts well to complex cases including revision surgery, severe deformities, and patients with prior incisions or soft tissue challenges. Be mindful of the infrapatellar branch of the saphenous nerve, which can be injured during the approach, potentially leading to postoperative neuroma formation. If inadvertently cut, the nerve should be resected and the end buried to reduce the risk of painful neuroma.
