Hello, Doctor. I am a student midwife from Algeria, and I have a question about perineal massage during labor. Could you please explain how perineal massage is performed during labor? During my clinical placement, I observed that some midwives insert the index and middle fingers of both hands until they reach the level of the ischial spines, then apply gentle pressure against the vaginal walls in a downward and outward direction to "create more space for the baby." They continue this maneuver until the baby is almost crowning. My question is: Does this technique actually facilitate the birth of the baby's head, and would it be considered a form of perineal massage, or is it a different maneuver altogether?

love your phrase, “walking the fine line of misinformation.” I was a medical librarian at Vanderbilt before I had my babies and stopped working for a while. I noticed that most misinformation wasn’t just pulled from thin air. It was usually based on some nugget of truth that someone misinterpreted, over-generalized, or misquoted from a study. Like the whole “cinnamon cures diabetes” idea. It’s not 100% made up BS; in some small, short term studies cinnamon supplementation has helped lower blood glucose in type 2 diabetes. The results aren’t consistent, it hasn’t been shown to help type 1, and you can’t stop taking your medication! But the snake oil people get ahold of this little piece of info and suddenly they’re hawking their cinnamon oil to people with type 1 & claiming they can stop using insulin if they take enough cinnamon oil. That’s the most dangerous kind of misinformation. It’s brilliant how you find the study they are probably trying to cite and then dissect what the evidence is actually showing us!

This highlights the need to allow women to labour in a warm bath to assist with tissue softening. Worldwide the midwifery model of care is shown to reduce episiotomy and have it used more appropriately.

Hey Doc can you talk about HIE? I’m seeing some videos of ladies giving birth in a hospital but allowed to labor for too long which caused their babies to pass away from HIE…. I’m due in Jan and I’m scared!
