Delirium tremens (DTs) is the most severe and life-threatening form of alcohol withdrawal, characterized by profound mental status changes and severe nervous system overactivity. It occurs when an individual with severe alcohol dependence abruptly stops or rapidly reduces their alcohol intake. DTs is a medical emergency that requires immediate hospitalization and medical detoxification. At the neurochemical level, chronic alcohol intake suppresses the brain's excitatory glutamate system and enhances the inhibitory GABA system. When alcohol is suddenly removed, this balance flips. The brain experiences a massive rebound of glutamate and a drop in GABA, resulting in dangerous central nervous system hyperexcitability and autonomic instability. DTs does not happen immediately after a person stops drinking. Instead, it follows a progressive timeline: The presentation of delirium tremens is defined by a distinct triad of severe neurological and physical manifestations. 1. Global confusion and deliriumThe hallmark feature of DTs is a sudden, fluctuating disturbance in attention, cognition, and awareness.Disorientation: Patients are often entirely unaware of the time, where they are, or who they are.Fluctuating alertness: Their mental state can rapidly shift between periods of lucidity, extreme drowsiness, and hyperactive agitation.Severe psychomotor agitation: Patients may become extremely restless, irritable, terrified, or aggressively combative.2. Vivid hallucinations and sensory disruptionsPatients experience sensory perceptions that feel entirely real to them.Visual hallucinations: The most common type, often involving terrifying imagery like seeing swarms of insects or small animals crawling on the walls or bed.Tactile hallucinations: Feeling things that are not there, such as bugs crawling on or underneath the skin (formication).Auditory disturbances: Hearing unexplainable sounds, threats, or phantom voices. 3. Autonomic hyperactivityThe hyperadrenergic state places extreme stress on the cardiovascular and regulatory systems: Severe body tremors: Violent, uncontrollable shaking that affects the hands and whole body.Tachycardia: A dangerously racing heart rate.Hypertension: A severe spike in blood pressure.Diaphoresis: Sudden bursts of heavy, soaking sweats.Hyperthermia: High body temperature or fever.
