In my version, they were infected right before starting their first shift at the hospital. This happens to almost every low-level position there. Doctor Harlow himself is behind it all..(I’ll make a comic about it too.) Some of the symptoms are hallucinations and intense hunger. Over time, the Intern has learned that coffee is a kind of a suppression for their symptoms. Agent and Intern have a good relationship; Agent has an interest in Intern, while Intern can't fully trust them for fear of being exposed, knowing what happens with every anomaly.(slide 10) Not everyone(in this case all Interns) is aware of their infection, and hiding their symptoms is very difficult, so each infected person is sent to special facilities.
