I could be wrong but I don’t think this is an official report. This is an incident report. This is what they write down when they get to you for information. It’s literally just what you say to them. For example, when a paramedic comes and asks what medication you’re on I could say I’m on ADHD medication (I don’t have ADHD) but they’re not checking it in the moment. They’ll just write it down.

It’s a discharge summary from an A&E visit and as someone who has typed many nhs documents the ‘apparently’ is the Dr indicating to all other health professionals ‘we are highly sceptical of this whole palaver and treat this whole thing with caution’ - like a raised eyebrow.
