This is one of the biggest problems with the US healthcare. In my country doctors don’t ask your history because they already know it. Every visit and every treatment since the day you are born is recorded in centralized database. A big advantage of public healthcare over fragmented private one

That was my dad right before his heart attack. he said no to all of the following, which should have been yes. 1. Don’t have a history of heart disease (yes—his father and all male uncles had heart attacks in their 50s? 2. do you smoke ? no (be he smoked and chews nicorettes gum at the same time—which is ill advised). they sent him home and he had a heart attack that night.

"just read the chart" I go in every week for an injection and I have been doing that for two and a half years, yet the nurses still ask me my name and birthday every time. It's a mandatory verification to make sure you are who you say you are, because if they screw up and give someone else my medication, they're fucked.
