So it sounds like this happened during veress needle entry at the umbilicus. This is a very common access technique but doing it in the midline makes me very nervous. I did that during training but have not done it this way for years. I usually chose a site in the left upper abdomen called palmers point to avoid this from happening. Usually the worst thing that can happen at Palmer’s point is the stomach or colon gets poked. I dont have any hard numbers but anecdotally feel that most general surgeons have moved away from veress access in the midline. It still seems very common for gyn to use this site. I am not disparaging any specialty, just a variation in practice i have seen. I was involved in an aortic trocar injury with this exact scenario. They called me for help as the patient was bleeding to death. Talk about going from 0-60 in about 2 seconds. It had a profound impact on me as a trauma and acute care surgeon, and i know the patients original surgeon was the second victim. Solidarity with you for sure but also thinking of the surgeon, it is a very lonely job that very few can understand

Sending you so much love and prayers! I had my aorta knicked at 18 when they took out my appendix. Thankfully you will be able to live a pretty normal life besides follow up ct scans and ultrasounds for the rest of your life. So happy that they found out about the tear sooner rather than later! ❤️
