So if something worked for 30% of people in a study, does that imply it works 30% of the time for everyone, or does that mean it works all the time for 30% of people (or somewhere in between, like works 50% of the time for 60% of people)? Depending on the answer, the drug that studies show is less effective might always be the right choice for certain patients (if you already know it works for them).

Omg I’ve always sworn Tylenol does absolutely nothing for my pain and people argue with me that it’s just as effective as ibuprofen and it’s just in my head. I feel SO validated right now.

what about powdered aspirin packets like BC packets? Post foot surgery they were far more effective than 800mg of ibuprofen for me. Just curious if you've read? Love ur page! (if youre too busy to see this, I am so okay with that) 😊

MD Pain physician here. look at the Oxford analgesia league tables. In blinded studies Opioids aren’t that great. Ibuprofen beats moderate morphine doses for dental pain. Ditto acetaminophen vs lower opiod doses. Opioids are really not that effective in blinded studies (massive placebo benefit) - but their benefit as an agonist you can just increase the dose as needed until either pain free or unconscious . NSAIDs block an enzyme meaning there’s a ceiling- once that enzyme fully blocked can’t block it anymore

Fun fact that she knows that you won’t know, cocaine is not illegal in every format. There’s actually a topical spray version of cocaine that we use in the operating room, and it’s a vasal constructor, meaning that it shrinks your blood vessel so you don’t bleed very much. We use it very, very often in surgeries in and around your mouth and throat because of how much bleeding happens in those areas. The good news is, that formulation of the drug is not something you could get high off of even if you misuse it. And, by the time it’s being sprayed on your throat, you’re definitely already anesthetized.

the menstrual pain one is hilarious bc the medication specifically marketed for it is literally acetaminophen and caffeine. 😂 but i’ve always found ibuprofen works better. i think ibuprofen is good for cramping/strain related pain as well.

I have never felt much of any effect from acetaminophen for headache or period cramps or anything whereas ibuprofen helps significantly, this is so validating. Why is it still legal and commonly used though if it doesn’t have much efficacy and causes a lot of harm across the population?

Acetaminophen also has a very close Therapeutic Dose/Toxic Dose gap, so even the "recommended safe dose" could potentially cause a lot of liver damage based on how the body metabolizes it - even without prior history of liver issues.

honestly I wish I could show this whole video to every medical profession I've ever seen. "have you tried Advil/Tylenol?" of course I've tried the very common OTC medicine! I wouldn't be here if it worked! and now I know I was right!

Living in Europe where I got screamed at by a doc for taking 800mg of ibuprofen and then told to take 1000mg of Tylenol every 6h 😑 bad doc aside, they're so delicate about ibuprofen here but tylenol is just the de facto treatment for everything

This is so interesting as an EMS worker! Anecdotally, our patients seem to get a much greater level of relief from IV acetaminophen compared to IV fentanyl or morphine or ketamine. However the greatest amount of pain relief was found to come from a combo of acetaminophen and ketamine. Of course, these patients had something more significant going on than just a typical headache or ankle pain, but still super intriguing how route of admin and possibly environment play a factor.

I would love it if you talked about why acetaminophen is paired with oxycodone for pain medication (percocet), and why that is prescribed for chronic pain over just a oxycodone of the same dose?
