You may have already tried it, but the only thing that has helped my Ava that had the same issues is a combination of Gabapentin & Cerenia 2 1/2 hours before we get in the car. I hope you find something that works.

I take my cat in a dog crate with a litter box in it when I take him to the vet. When he was a kitten I used to bathed him and blow dry him. He loves the blow dryer every time I dry my hair he wants me to blow dry his as well.
