Slip vs square knot is all about tension. Tension on 1 hand = slip. Equal tension on 2 hands = square. Like u said, nothing to do with direction of throw - most commonly taught error on the surgical rotation in med school

Anyone who works in cardiac surgery NEEDs to know how to tie a slip knot and how to lock it! Don’t forget to push the knot down, not pull on the “post”- just keep a bit of tension on the post, otherwise you’ll rip the tissue

capsized is also called a half hitch , you want a slip knot because you don't want an air knot. trying to ligate a vessel ? do an air knot and you don't ligate it ...
