@askpaindoc: The 0-10 pain scale is one of the most commonly used tools in medicine. The problem is that acute pain and chronic pain don’t always fit into the same framework. Someone with acute pain may rate a 7/10 because they’re experiencing a sudden injury or illness that completely disrupts their day. Someone living with chronic pain may also rate their pain a 7/10, yet still be working, taking care of their family, or making it through the day because they’ve spent months or years adapting to it. That doesn’t mean they’re suffering less. It means they’ve learned how to function despite it. This is why pain scores should never be interpreted in isolation. The number matters, but understanding how pain affects someone’s ability to work, sleep, exercise, socialize, and enjoy life often tells us far more than the number itself. As a pain physician, one of my most important questions isn’t just “How bad is your pain?” It’s “What is your pain preventing you from doing?” That’s often where the real story begins. I’ve gotten into so many arguments with specialists, physiotherapists, peers, like you guys really need to stop using the pain scale. Can you listen to what I’m telling you it is not the same. I can’t explain it. Let me just tell you what I now can’t do based on my pain level, well you’re sitting here you’re fine, really 10/10 would be I’m calling an ambulance for you. No ten is me pacing at night because my body can’t sit still 10 is me crawling up my stairs but 10 is my every night. My Pain baseline is six. I’m constantly fidgeting. I have to keep moving. I laugh and over talk to distract myself and even as much as I talk, nobody listens. It’s the exact same as blood pressure oh your blood pressure is fine. You’re not in pain, really you tell me, you’re a nurse just like me. Maybe you should do some education, acute pain may show someone’s blood pressure skyrocket someone with chronic pain, I almost feel like we’re athletes where our breathing doesn’t increase our heart rate doesn’t increase we are conditioned by pain, my blood pressure is not a reflection of my chronic pain so I tell my peers you need to do some investigation. You need to do some research on what chronic pain is like with vital signs compared to someone with acute pain. Both should be removed in assessing chronic pain.

Absolutely! I wish more healthcare professionals focused on the functional limitations caused by the level of pain rather than reducing it to a number. The throbbing is so bad it is causing my eyes to water and I’m unable to stand up is going to tell you a heck of a lot more than It’s a seven.

Thank you for this video. I show these to my Primary Care doctor. 5 to 6, I can still function, but I’m limited on what I can do. 7 to 8, my life is halted. 9 and 10, that’s an extreme flare and I end up in the ER.

Those pain scales aren’t even close to accurate. My old 8 is now a 3. Most people would go to the emergency room at my 4-5 now.

my pain is from a 3 to 10. It all depends on the day. most of the time Im about 5 to 6 but a good day is 3/4. BTW when Im a 7 to 8 I'm in bed doing nothing at all. But a 9/10 I'm crying and can't move at all and don't can't do nothing so I have to suffer.

I had a nurse tell me if I could talk, then I wasn't in that much pain. I live at a 3-5 baseline. I was at a 10 because they were 5 hours (at that point) late giving me my meds. I ended up waiting 14 hours for my meds.

so my meds xtampza er and dilodod po take pretty much care of my day to day pain but every cycle i go into a flare that sends me to the er which i have to get iv dildod and toradol to get me back to a controlable number but now my dr is talking about a pain pump im nervous because i need a way to give myself more meds safely if i have a flare up im tired of going to the er
