For everyone that is too lazy to search basically jimmy smiths (miguel prado) accidentally picked up a real knife while filming the scene where he kills someone with dexter, and he stabbed the stunt actor with the real knife, only reason the stunt actor survived is because of the layers of plastic, also the reaction that the stunt actor gives in the scene is real because he was tryna warn them but he couldnt bcs his mouth was duct taped.

During the filming of Season 3, Jimmy Smits accidentally picked up a real knife instead of the prop knife during a stabbing scene. He stabbed stunt performer Jeff Chase in the chest, but Chase was wearing a small acrylic protective plate over his heart. By extraordinary luck, the knife struck the edge of that plate, and he escaped with bruising rather than a life-threatening injury. Michael C. Hall reportedly realized what was happening and shouted for Smits to stop, but the action happened too quickly
