As a former L&D nurse and a NICU nurse, I have strong opinions about free births. I have heard the argument that women have been giving birth for thousands of years outside of hospitals but I never hear about maternal and neonatal mortality in the same discussion. I worked with a nurse who had a free birth, limited prenatal care with a lay midwife. Her labor went horribly wrong and she finally agreed to be transported to the hospital (the RN in her kicked in). She had an emergency C/S and her baby had an undiagnosed prenatally, LARGE diaphragmatic hernia. If diagnosed early in pregnancy, she could have had a fetal surgery repair but instead, the baby died on ECMO. And this is not the only home birth nightmare I have witnessed.

Hemorrhaged will all 3 of mine. With the 2nd and 3rd had a protocol in place to prepare and still almost died both times. It can go bad SO fast. It's so sad to me that people take thus risk.

I’m a nurse-midwife (DNP) and I attend planned home birth. I support a person’s right to choose where & however they want to birth, but don’t personally recommend unassisted free birth. The entire thing is about accepting radical responsibility & not blaming someone else when bad things happen. So it’s a very fascinating discussion to look at if the FBS is indeed responsible for its teaching about accepting radical personal accountability. But the root issue to examine here is WHY. Why would these families risk literal death over having an attendant or going to the hospital? Spoiler alert: it’s not bc they don’t care if their baby dies. The entire OB medicolegal system needs to take a hard look at themselves & their role in pushing people to this extreme. However, we are a very long way from that reflection & transformation of the system/culture.

We run into this issue with lay midwives too, not recognizing crisis fast enough and when they bring them to the hospital they are in TERRIBLE shape, where if they were in the hospital, it would have been caught quicker.

As an OB nurse of over 20yrs, I am just taken aback by it all. I listened to the podcast and sat with my jaw dropped and shaking my head through much of it. And then to hear that Emily (I believe) actually had a stillborn I wondered if she would change her ways. I do not believe anyone without some type of medical training should be giving medical advice. It breaks my heart for those that listened to them and then lost their babies. Those deaths were unnecessary.

The Birth worker is the same as a Doula they can’t do any risk and benefit conversation, they can’t advise or make recommendations, they are like a friend but I guarantee families see them as an expert and look to them for guidance and tbh they prob give it. I think Doulas should require some type of professional supervision that way they are connected to midwife’s or something along those lines to make sure that person can truley educate parents

It’s sad that there is so much mistrust of hospital L&Ds that women are willing to risk their baby’s (and their) lives to have a home or free birth. I feel that these women are given an unrealistic explanation of not only WHAT we do, but WHY we do it.

Worth noting that home birth and free birth are different, many people get them confused in this context. You can have a home birth with midwives + specialists. Also most doulas know their scope which is to be spiritual/emotional support while a trained professional handles the medical side.

I view this the same as I see antivaxers, drunk drivers, and such. They willingly endanger themselves and others knowing full well the risks. Medically speaking, if they're not regulated and they make it clear that their training isn't locally or federally monitored then they have no one but themselves to blame. Just because it's run by a "former" dollar, doesn't mean it's remotely reliable.

It’s a symptom of a broken maternity system that’s causing unnecessary harm. Doulas and birth attendants need to stay in their lane of emotional support if the mother. Medical procedures should alive done only by the appropriately trained person. The problem lies when greedy people misrepresent what they do by inferences they are medically trained or the mother doesn’t take responsibility for her choices by being fully informed. Radical responsibility isn’t accepting death as part of the plan, radical responsibility is being totally knowing about your options then choosing what’s right for you. Free birth isn’t bad, it’s an option outside of a very broken system and the more it’s vilified the more it will be used. You should know all your options when you birth in hospital as well. Private midwives in Australia are the gold standard of care to birth attendants home safely.
