Dealerships technician here, new guy drained the transmission instead of the oil on a 23 porsche macan overfilled the oil and then drove it. I don’t know how much it was exactly but then he proceeded to destroy a 30k tire machine. Still not fired btw

Mars Climate Orbiter. In 1999, the $127 million spacecraft was destroyed because one engineering team calculated force in English/Imperial units (pounds) while another used metric units (Newtons), causing the probe to burn up in the Martian atmosphere.

personally my dad cut through the fiber optic for South Auber cable in Washington State, he called 811, it was marked incorrectly, saved his ass. his stump grinder only cut 18 inches and the fiber was marked at 36 inches. a lot of football fans were upset that playoff weekend. he was proud to have not been on the hook for the bill.

Well this basically the locomotive was operated by mean a few other volunteer for an a active period on the Keighley Worth Valley Railway. and the left cylinder completely renaded itself because of one of their volunteers letting too much water creating pressure to build up in the cylinders while opening the cylinder Cox cosmith grenade inside of itself best shredding that actual internal cylinder and piston completely that is half a million just to replace raw cast iron compound cylinder they're not cheap in their very expensive since no company mass produces them anymore and we have to Kevin custom fabricated so a complete custom order.

my brother sent me this photo of a privately owned Canadair CF-116D Freedom Fighter which caught fire after a fuel leak in the engine ignited during rollout, about 3-10 million in damages apparently

A nearby farm had an employee that dropped a cigarette outside a barn that had about 60,000 bales of hay in it. Don’t know the exact number but was about 1.5m in product loss and property damage

I went to a museum and they didn't block all the controls and the apu was on for some reason I hoped in the plane I new the sequence to put down the wing and half start the engine for the plane and I nearly started moving and cause I put the wings down and the museum was small and there were a bunch of hanging planes the wing hit one and it fell and ummm yeah

A co-worker ran a construction jobsite downtown, he/we were responsible for some of the water installation. He ran that job for over a year, the entire length of the big job, 3 floors of s commercial high rise downtown of a MAJOR North American city. (I'm not saying which city, or country) The water was under 150psi (2.5 times working pressure) air pressure test for 2 weeks, under water pressure for 3 months and one sunday a security guard making his rounds found the floor was wet on the 6th floor. He traced it back to the elevator shaft and started making calls. The calls probably took a couple of hours, but a service tech of ours went in a couple hours later and the water was at least at the 4th floor, he traced it back to a specific line and shut down that line, and EVERYONE WAS CALLED. A 2 1/2" fitting came apart on the 17th floor, and (supposedly) hit every floor below it. I was told multiple estimates between $5 and $20 million. I was also told, EVERYONE who wanted a new desk or laptop put in a claim from the 1st floor to the 17th. The company kept him on as, despite, this happened on his watch, everyone who had a free day or 2 or week were sent there to keep busy and it would've been impossible to determine who worked on what where? Last I heard he still works there to this day. Good guy! Very good Foreman and person.

I towed a brand new, souped up ford mustang that the service tech decided to show off to his gf before the customer took ownership of it. Blew the motor, transmission, bent an axle, and left a trail of fluids about 1/2 mile down the road. About $120k after all was said and done. Another time, another towing service was using a rotator to fish a semi tractor out of a river and they didn’t do it right, so they laid down a $300k+ truck into the same river the semi tractor was in. I don’t know the dollar amount in damages, but the historical value is priceless. Years ago, I helped pull a fire engine out of a river. It was filling up from the river on the boat launch and supposedly the brakes failed and it rolled down the boat launch. After we were on scene, we discovered that that engine was from New Jersey, and it was at Ground Zero of 9/11 to assist the NYFD.

In 2019, at the start of my shift at Honda Manufacturing Of Alabama, I was writing the starter shift notes and dropped my ink pen perfectly into the small Line track. It broke the chain. Cussed Honda around $21 million in lost revenue. All because of an ink pen. They transferred me.

Older gentleman (like 87) mistook his gas pedal for the brake... at a Mercedes dealership while I was in Germany. He wrecked a showroom window, 7 cars and 1 trailer. One was a brand new Lamborghini. I think the beginning estimate was 800k.
