First Week After Hip Replacement: What To Actually Expect The first week after #hireplacement is the stretch patients ask me about most, and it sets the tone for everything that follows. You stand and take your first steps within hours of surgery, with a physical therapist beside you and a added stability from a walker. Most of my patients go home the same day. Early movement is how we prevent blood clots and stiffness. . Days 1-7 come down to three basic things: controlling pain, managing swelling, and protecting the incision. PAIN should be real but manageable, and it should ease a little each day. We use multimodal pain control, which means acetaminophen, an anti-inflammatory when it is safe for you, a nerve block, ice, and elevation working together so we lean on narcotics as little as possible. Take your medication on the schedule we give you rather than waiting for pain to spike. MOVE every hour you are awake. Short walks around the house beat one long walk each day. Swelling and bruising are expected and can come and go for months, so ice the hip, elevate above heart level, and avoid sitting in one position too long. Keep the incision clean and dry until we clear you to shower. CALL the office the same day for calf pain that does not ease with rest, spreading redness, drainage past the first few days, fever with chills, or a sudden change in how the leg bears weight. Chest pain or shortness of breath means calling 911.
