EM Pearl: Clonidine overdose mimics opioid toxicity — presenting with bradycardia, hypotension, miosis, and CNS depression — and is a classic toxicologic "mimic" tested on EM boards. The key distinguishing feature is that naloxone may partially reverse clonidine's CNS depression but will not reliably reverse the bradycardia or hypotension, which are driven by central α₂-agonism reducing sympathetic outflow. Pediatric ingestion of even a single tablet can cause life-threatening toxicity, making clonidine a "one-pill-can-kill" drug in children — another high-yield boards concept (and one we have other podcast episodes on).
