First actor to request the change was Peter Davison, and it remained that way with only series 1 ever carrying the credit Doctor who since, Tennant then requested the change back

tell that to how the character was credited for most of the classic run, almost the entire target novel range, the war machines (doctor who is required), the underwater menace (where he signs his name as Dr w), and of course the serial "Doctor Who and the Silurians". If you're gonna get fanboyish about it know the lore.

Well technically They usually go by the Doctor but they have been called both in-universe, in the show’s credits, in titles, and by people who work on the show. Out-of-universe, Doctor Who is less ambiguous.
