In 2024, Stanford researchers published a study in Cell on a molecule called Xist, which only female (XX) cells produce. Its job is to quiet the second X chromosome, but in doing so it forms complexes that can trigger the immune system to attack the body, offering a real biological reason four out of five people with autoimmune disease are women (NIH). The team noted that standard tests for this were historically built on male cells, which don’t make Xist, so a major source of women’s autoimmune antibodies was essentially invisible to the test. Pair that with the documented gender pain gap, where women with the same chest pain as men are more likely to be handed a mental health diagnosis than checked for a heart problem, and you can see why “it’s anxiety” got repeated for decades. It explains the autoimmune female bias specifically, not every condition in the cluster, and the human side is still being studied.
