Closing on the saliva ejector creates negative pressure and can lead to backflow. There are backflow protectors available but not all offices use them. I never tell my patients to close, with or without a backflow protector, it’s unnecessary.  Dental professionals are trained on maintaining the lines, running the lines in between patients and at the end of the day.  The CDC states, “Although no adverse health effects associated with the saliva ejector have been reported, dental health care personnel (DHCP) should be aware that backflow could occur when they use a saliva ejector. DHCP should not advise patients to close their lips tightly around the tip of the saliva ejector to evacuate oral fluids. DHCP should contact the manufacturer of the dental unit to review proper use and maintenance procedures, including appropriate cleaning and disinfection methods.” Dental offices should be running their dental water lines in the am, in between pts, and at the end of the day, we should also be shocking our systems, in addition to testing our dental unit waterlines, and changing our traps.
