While flying from Denver to Chicago, the aircraft's tail-mounted engine exploded, sending metal fragments through the tail section and severing all three independent hydraulic systems—an event engineers believed was virtually impossible. With no functioning flight controls, Captain Alfred Haynes, First Officer William Records, Flight Engineer Dudley Dvorak, and off-duty DC-10 instructor Captain Dennis Fitch faced an emergency unlike anything seen before. Unable to steer the aircraft normally, the crew devised an extraordinary solution: controlling the plane using only the thrust from its two remaining engines. Against incredible odds, they guided the crippled DC-10 towards Sioux Gateway Airport in Iowa. Moments before landing, the aircraft rolled sharply, struck the runway, cartwheeled across the airfield, and burst into flames. The crash claimed 111 lives, but 185 passengers and crew survived, making it one of the most remarkable survival stories in commercial aviation. Investigators later determined that the disaster was caused by a microscopic manufacturing defect inside the titanium fan disk of the failed engine. The lessons learned from United Flight 232 transformed aviation safety worldwide.
