@amhbhaiofficial: How to Suspect Typhoid, Malaria & Dengue from CBC, Step by Step Step 1: Check WBC count. First look at the total white blood cell count. In typhoid, WBC may be low or normal, sometimes with relative lymphocytosis. In malaria, WBC may be low or normal. In dengue, leukopenia, or a low WBC count, is common. Step 2: Check platelet count. A reduced platelet count can occur in all three diseases, but it is particularly important in dengue and malaria. A progressive fall in platelets with fever strongly raises suspicion for dengue, but it does not confirm it. Step 3: Check hemoglobin. In malaria, hemoglobin may decrease because malaria can cause destruction of red blood cells. In typhoid and dengue, hemoglobin is often less useful for distinguishing the disease. Step 4: Check hematocrit (HCT/PCV). In dengue, a rising hematocrit together with falling platelets can suggest plasma leakage and requires medical attention. Hematocrit alone cannot diagnose dengue. Step 5: Look at the overall CBC pattern. Low WBC + low platelets can suggest dengue, while low platelets + anemia may raise suspicion for malaria. Low/normal WBC with fever can occur in typhoid. However, these patterns overlap with many other infections. Step 6: Confirm with specific tests. CBC is only a screening clue. Typhoid: blood culture is preferred. Malaria: peripheral blood smear or a malaria rapid diagnostic test. Dengue: NS1 antigen or PCR during the early phase, and IgM later in the illness. Important: Never diagnose typhoid, malaria, or dengue from CBC alone.
