So the RPG they used was a "re-militarized" one. meaning that its internal blast chamber for the propellant charge originally got cut open to render it unusuable (de-militarize). the hole was later welded shut to make the RPG usable again (re-militarize). the weld however was not properly done, causing it to blow open when they shot the RPG.
