Researchers keep finding the same surprising thing: the shingles (herpes zoster) vaccine is associated with more than just protection from shingles. In a 2025 Nature study, scientists used a quirk of Welsh policy as a natural experiment. Eligibility for the vaccine was set by exact date of birth, so people born just before vs. just after the cutoff were nearly identical groups, separated mostly by chance. Those eligible for the vaccine had roughly a 20% lower probability of a new dementia diagnosis over 7 years, which is far stronger evidence than typical “vaccinated people are healthier” studies, because the groups were comparable. A separate large cohort (174,000 adults) also linked the shingles vaccine to about a 25% lower risk of heart attack and stroke, lower risk of blood clots, and lower death rates. Nobody fully knows the mechanism yet. One theory is that preventing the virus from reactivating protects the brain and blood vessels; another is that the vaccine’s adjuvant (the ingredient that boosts immune response) has its own effects, since a similarly adjuvanted RSV vaccine showed a comparable dementia signal. Honest notes: most of this research is in older adults (the people who get shingles and the vaccine), some of it is still being peer-reviewed, and association is not the same as proof of cause, though the Welsh natural experiment gets unusually close.
