If you were not liked do not bother with this. I had one internship where they were clearly disappointed in me and I was the wrong hire and they made no effort to hide it. I had one coworker I liked who I used as a reference later but I was not going to pretend that I was grateful that they had treated me like scum for a whole summer

Also write down phone numbers of people you will write down as future references. An email address works like you said, but if a future employer is seriously considering you, they may make a phone call instead of an email.
