Check kidney function (eGFR) before starting and periodically during treatment. Do not initiate if eGFR <30 mL/min/1.73 m²; if eGFR is 30–44, initiation is generally not recommended. Advise patients to report symptoms of lactic acidosis (extreme tiredness, muscle pain, trouble breathing, severe weakness). Metformin rarely causes hypoglycemia when used alone, but the risk increases when combined with insulin or sulfonylureas. Long-term use may reduce vitamin B12 levels; periodic monitoring may be appropriate.
