Whats a dysplastic naevus? Its the fancy pants medical way of saying your mole is slightly odd in nature! When it is really odd they end up calling it a melanoma but sometimes it just doesn't meet criteria to be called melanoma so they call it dysplastic. Low grade dysplasia means the pathologist is patting me on the head and giving me an "Attaboy" for finding the weird mole BUT also tellign me it isn't a melanoma. A severely dysplastic naeuvs I imagine is them telling me it is sooo close to being called a melanoma but not quite - I tend to have a discussion with the patietn and offer to treat like a melanoma in these instances. Some patients take me up on the offer and others choose to monitor these scat sites. A moderately dysplastic naevus is one of my least favourite diagnosis in medicine and often leads to me asking for a second opinion and further review of the specimen!!! Some caveats - None of this matters if you have only partially sampled a spot - hwo you choose to biopsy is hugely important and donig a partial sample of a large abnormla lookign spot and getting back a severely dysplastic naevus is not the reassurance I am looking for. There is also large variability between pathologists when reporting these things - one pathologist calling something a melanoma and another pathologist calling the same thing a dysplastic naevus is NOT unusual and just highlights how tricky melanoma diagnosis is when it is in its early stages!
