Spider naevi, palmar erythema and gynaecomastia all track back to circulating oestrogen the failing liver no longer metabolises. Each is non-specific alone, spider naevi appear in pregnancy and in around 10% of healthy adults, roughly a third of cirrhotics have them, so it’s the cluster that carries the weight.
