My take is that it's just muscle memory. Ned forgot Peter, but that doesn't mean every learned habit disappeared. The handshake could've stuck even if he doesn't remember why he knows it.

let me break it down rq. It wouldn't explain how he knew the handshake. his muscle memory kicked in, making something flicker in his head, as some of the memories of his old friend return. He didn’t remember everything, he just remembered he was best friends with a similar guy that has the same name. Also he possibly remembered he was the guy in the chair so yeah. also even if it's muscle memory you don't just remember it that fast. It's like playing guitar before and didn't play again after a few years. You don't easily remember something like that but you still have a few memories "muscle memory". But Ned's case is different somehow something clicked inside him and did the handshake. You just don't do that and say he didn't remember anything. He wasn't going with the flow either since it clearly shows that ned knows the handshake.
