In a healthy vagina, protective Lactobacillus bacteria keep the environment slightly acidic, which prevents harmful microbes from overgrowing. When this delicate balance shifts, anaerobic bacteria multiply rapidly and produce compounds known as amines, which release a strong, fishy scent that often becomes more noticeable after washing with alkaline soaps or having intercourse. This odor is frequently accompanied by a thin, watery, off-white or grayish discharge, though it typically does not cause severe pain or itching. Fortunately, bacterial vaginosis is straightforward to diagnose and treat once evaluated by a healthcare provider. Treatment typically involves a short course of targeted antibiotics, prescribed either as an oral medication like metronidazole or as a topical cream or gel inserted directly into the vagina.
