Spoiler: The real clue is in Peter's letter to MJ. There is a line where he essentially says, even though you can't remember me I hope that deep down in your heart you know that you do love me. In Ned's case the handshake was about muscle memory. And his "Peter" surprise was Peter is Spider-Man not an unlocking of his old memories. This refers back to MJ mentioning to Peter that Ned can handle knowing his identity and what happened. And MJ wasn't mourning that's why she was smiling. She doesn't remember the memories she remembers the feeling. 💙❤️

there's a Thor Ragnarok reference in the movie as well, if you didnt notice...in Thor Ragnarok Bruce banner said he had a weird feeling like he knew valkyrie but didn't know, but when he transformed into Hulk she knew the Hulk....so come back to the fight scene in Brand new day...Bruce doesnt remember who Peter was because strange wiped everyone's memories but I the fight scene with hulk..Hulk remembers who Peter was, that's why the Hulk stopped himself from killing spider-man!!
