Honestly with that textbook presentation I’d shotgun ANA/antimitochondrial/sjogren labs and get early GI/hep referral. Get the ball rolling.

I initially thought that the itching was due to c holestasis, which was further proven with the elevated alk phos. I just got the fact that the patient has Sjogrens d/t the dry eyes and mouth but like given the definition of primary biliary cholangitis, wouldn’t ALT and ASP be elevated, at least mildly, since its injury to the liver ducts? Please correct me if I’m wrong in thinking about it this way

You described my mom to a T. As soon as you said stress and itching I knew it. She had PBC and succumbed to it after a failed liver transplant.

Itching is very specific for gallbladder crystals and stones especially when you combine it with icterus but normal transaminases🥰

Good job. Elevated ALP even with no symptoms, send the AMA. Urso can cause diarrhea-recommend Imodium for better adherence. Rpt labs q3 mos. A lot of non-responders and some other options available.

I had a patient with the same diagnosis but presented completely asymptomatic!! The only abnormal thing was her elevated ALP… after multiple negative imaging later, finally got diagnosed with PBC thru biopsy. And she was seronegative for PBC-specific antibodies… pretty cool!
