As a nurse, he is not cognitively ok. Id be worried about multiple small infarctions. He desperately needs a brain scan, and also a wife who takes the warning signs seriously instead of brushing it off as “he’s just dumb”.

As someone that has administered hundreds of cognitive screeners, this would absolutely indicate needing to be seen by a doctor. It doesn't have to mean dementia. It can be a reaction to medication (even one that hasn't had any side effects before) or a sign of hidden infection.

You should google the whole clock test thing because his multiple attempts before you gave him a hint about where the numbers should be is EXTREMELY concerning. You should repeat it in a day or two in the evening. Ask him to do it again. And this time - no hints.
