@dawson_.sheehan: Benzodiazepines are one of the highest yield drug classes in pharmacology, and almost every exam question traces back to a single mechanism. Benzodiazepines bind the GABA-A receptor and potentiate GABA, the primary inhibitory neurotransmitter in the central nervous system. Alcohol acts at that same receptor, which explains both why benzodiazepines are used to manage alcohol withdrawal and why combining the two produces profound central nervous system depression. The class produces five predictable effects. Anxiolysis, sedation, anticonvulsant activity, muscle relaxation, and anterograde amnesia. The amnestic effect is therapeutic rather than adverse, which is why midazolam is used before procedures. Three agents cover most of clinical practice. Midazolam for rapid onset and short duration in procedural sedation. Lorazepam for its absence of active metabolites, making it preferred in hepatic impairment and a first line agent in status epilepticus and alcohol withdrawal. Diazepam for its rapid onset and prolonged half life, which leads to accumulation in older adults. Respiratory depression remains the priority concern. Assess respiratory rate and depth, sedation level, and blood pressure. Concurrent opioid use carries a black box warning and significantly increases mortality risk. Flumazenil is the reversal agent but should be used cautiously in chronic users because of the risk of precipitating seizures. Benzodiazepines should never be discontinued abruptly. Withdrawal can produce seizures and can be fatal, so a taper is always indicated. In older adults, remain alert for falls, delirium, and paradoxical agitation. Save this for your pharmacology exam.
