HULK didn't want to come out because he didn't want to be summoned like a pokemon for battle. They only brought him out for fighting. He didn't like or want to do that.

They do Hulk a huge disservice by ignoring the internal struggle between banner and hulk. Its main reason for the strength loss between movies but its always passed over completely or edited out.

it would've been so badass if the hulk buster fell and when ogre were about to kill Banner, a green arm would come from the ripped out arm hole and punch the ogre away, then, Hulk would rip the hulk buster apart and come out of it.
