Every hematologist knows this visit. The patient is exhausted. Foggy. Short of breath on the stairs. They've been told for months, sometimes years, that their labs are normal. Then we look closer. Their ferritin, the body's iron savings account, is sitting in the teens. Technically "normal" on a lot of lab reports. Functionally, running on empty. This week, that story got a new ending. On September 16, 2026, the American Society of Hematology published new guidelines raising the ferritin cutoffs for diagnosing iron deficiency: Adults, including menstruating and pregnant patients: 30 ng/mL or lower. Heavy menstrual bleeding or anemia in pregnancy: 50 ng/mL or lower. Children 9 months to 4 years: 20 ng/mL or lower. Adults with inflammation, including cancer, IBD, or infection: ferritin under 100 ng/mL or transferrin saturation under 20%. That last one matters to me every day in oncology. Inflammation pushes ferritin up and hides an empty tank. Many of my patients have been told their iron was fine when it wasn't. The panel built these recommendations from systematic evidence reviews, not guesswork. And the gap they're closing is real. Up to 70% of iron deficiency in high-risk groups goes undiagnosed. Some people who were told they were fine now meet the definition of iron deficiency. The patient was never the problem. The cutoff was.
