Ovarian reserve and egg quality are NOT the same thing, and this is one of the most important distinctions to understand when we talk about fertility. Your ovarian reserve refers to the quantity of eggs remaining within your ovaries. Women are born with a finite number of eggs. That number naturally declines throughout reproductive life, and we currently don’t have a test that can simply count exactly how many eggs you have left. Instead, we use markers to help us estimate ovarian reserve. One of the most commonly discussed is AMH, or anti Müllerian hormone. AMH is produced by cells surrounding developing follicles within the ovaries, so the amount detected in your blood can give us information about the remaining follicular pool. An ultrasound looking at your antral follicle count, or AFC, can provide additional information about ovarian reserve. But this is where there is a really important distinction. AMH tells us primarily about egg quantity. It does not directly measure egg quality. A low AMH does not automatically mean the eggs you have remaining are poor quality. And a high AMH does not guarantee that your eggs are good quality. Egg quality is a different concept. When we talk about egg quality, we’re talking about the biological competence of the egg, including whether it can mature appropriately, fertilise and contribute to the development of a chromosomally competent embryo. One of the strongest predictors we currently have for egg quality is age, because the likelihood of chromosomal abnormalities within eggs increases as we get older. This distinction becomes particularly important when someone receives a low AMH result. I don’t want someone walking away from that pathology result believing they’ve been told they have poor quality eggs. They haven’t. A lower AMH may tell us that ovarian reserve is lower and, particularly in an IVF setting, it can help predict how someone may respond to ovarian stimulation and the number of eggs that may potentially be retrieved. But it cannot tell us whether an individual egg is capable of becoming a healthy embryo. And fertility is much bigger than AMH alone. We also need to consider ovulation, sperm health, tubal factors, endometriosis, reproductive hormones, thyroid function, metabolic health, nutrient status and the broader clinical picture. One pathology marker should never become your entire fertility story.
