When I was a newer nurse there was a lawsuit from a husband after his wife died from a blood clot. My note was pulled where I charted she refused her TEDs and SCDs and quoted where HE told me to “get the fuck out of the room so they could sleep” when I attempted to educate them more. I had her a couple days and charted again she refused them and to get out of the bed. The suit was dropped before it went to court.

In nursing school you're taught that your documentation is your legal defense in a court case. I had a manager tell me to shorten my narratives and to not include direct quotes from patients. She's no longer my manager.

I always tell new nurses to PETTY CHART. I will chart the dumbest shit becuz I’ll be damned if a doctor, patient, or coworker tries to make me lose my license. I put names, exact times, location and level of care. I DO NOT CARE!
