Research has identified an inflammatory subtype of major depressive disorder. A 2019 meta-analysis (Osimo et al., 37 studies, 13,541 patients) found 27% of depressed patients have low-grade inflammation (CRP >3 mg/L) and 58% have mildly elevated CRP. Pro-inflammatory cytokines (TNF-alpha, IL-6, IL-1β) can cross the blood-brain barrier and activate the kynurenine pathway, diverting tryptophan away from serotonin synthesis. A 2024 review in Cells (DOI: 10.3390/cells13050423) outlines how peripheral and central inflammation, combined with gut microbiota dysbiosis, contribute to depression through impaired neuroplasticity and serotonin disruption. A 2024 protocol study in Brain, Behavior, and Immunity describes ‘immuno-metabolic depression’ as a distinct subtype with characteristic symptoms (increased appetite, weight gain, fatigue) and elevated inflammatory markers. Important: Most depression treatments still target neurotransmitter systems and remain effective for many patients. The inflammatory subtype is emerging research that runs alongside the established model, not instead of it. Anti-inflammatory drugs are not FDA-approved as antidepressants.
