Probably reversed the Flo. There’s a lever and if turned the wrong way it will pressurize instead of vacuum. I ran a vac truck years ago. Used to reverse for a few seconds or so to dislodge clogs in the line. But don’t leave it too long

UPDATE: Preliminary information suggests the explosion may have been caused by MDI (methylene diphenyl diisocyanate) polymerization, potentially triggered by moisture or an improperly cleaned tank between loads. The rear clamshell door was reportedly found nearly 500 feet behind the truck. The investigation is still ongoing, and I'll share more as official details are released.

The fumes from the product you are vacuuming can often times ignite from a hot blower. Anytime I've loaded flammable or explosive material you perform a hot load which means you don't typically run the blower much if at all. You put vacuum on the tank prior to loading and shut in and isolate the tank and just let the vacuum in the tank load without the blower.

Definitely looks like chemical reaction inside the tank and definitely takes a lot for the back door to blow off and push the whole truck forward and I remember correctly the door blows off at 26 sum psi dosent sound like a lot but it’s the volume bet the psv wasent working properly or the isolation was closed depending how fast the reaction did happen
