The tactic I’d recommend:
Minor issue = no letter. Being awkward, forgetting something small, having a different opinion, taking a little too long to text back, etc. People are human.
Meaningful screw-up = 1 letter. Disrespect, inconsistency, lying about something meaningful, repeatedly breaking plans, pushing a stated boundary, disappearing and reappearing when convenient, being deliberately cruel during an argument, etc.
Serious pattern = 2 letters. The same behavior happening again after it has already been discussed. At that point, you're not dealing with a mistake; you're getting information about the person.
Fatal error = whole name. Some things shouldn't earn multiple chances: violence or threats, coercion, stalking, deliberately violating consent, serious manipulation, dangerous behavior, or anything that makes you feel genuinely unsafe. You don't need to finish the game to block someone.
