Among the available treatment modalities, Ultrasound-Guided Foam Sclerotherapy (UGFS) has become an essential, minimally invasive procedure for managing varicose veins. UGFS involves the injection of a sclerosant foam directly into incompetent veins under real-time ultrasound guidance. This foam displaces blood within the vein, allowing intimate contact between the sclerosant and the vein endothelium, causing controlled endothelial damage, vein closure, and eventual fibrosis. The advantages of UGFS are substantial. It can be performed safely under local anesthesia on an outpatient basis, has a low complication rate, and offers excellent cosmetic and symptomatic results. It is particularly useful in patients who are poor candidates for surgery or endovenous thermal ablation, or those with recurrent varicose veins. Post-procedure compression and early ambulation are important for optimizing treatment success and minimizing complications such as thrombophlebitis or hyperpigmentation. Although UGFS is effective and well-tolerated, careful patient selection, proper foam preparation, and skilled ultrasound guidance are critical to maximizing efficacy and safety. Rare but possible complications include allergic reactions and transient neurological symptoms, which necessitate thorough pre-procedure evaluation.
