Just know if the punishes kid didnโt die he would be as old as Peter which is one of the reasons why heโs devastated in the hospital, also he says something like โyouโre kid I always thought youโd beโ or something like that and it could be because thatโs what he thought Peter would look like but it could also be cause Peter reminds him so much of his son just older

Personally I believe the doctors saw his face same as the people in the train saw Tobey, when Peter leaves the hospital just before Frank is visible in the window the doctors at the door talk to each other and look directly at Peter. The medical staff and maybe Miles mother, will keep Peters secret even if they don't know his name they know his face.
