As a person with over 20 years in EMS, our entire healthcare system is broken and has been for a long time. People who need and ambulance can’t afford the bill, while people who use as a taxi and never pay, use it weekly. Many people that just need an urgent care, go to the ER. And don’t get me started on my insurance increases over the years.

Dr. Primus, My wife and I are also small private practice owners. While I agree with you on most, I also have concerns about not-for-profit healthcare. Removing profit can have some upsides but there are also downsides. Often you remove an incentive for efficiency. See Medicare. There are two main factors in the difference between health care in the US and other countries that you have not seen you touch upon yet. First, Americans believe their premium should be $500 a month but get $1,500 a month in benefit. That just doesn’t work. Insurance is pooled risk (see auto insurance). Not pay a monthly premium and get everything covered. The second is the attitude towards healthcare. Americans will spend whatever it takes to have their 90-year-old grandfather’s hip repaired. Even if it is the hip surgery that kills him. This is a very difficult topic but there is a large difference in the attitude towards healthcare in the US to those other countries that you mention. I do love the idea of a for-profit company having to provide a basic plan for no profit. Then they can charge for upgraded plans. Keep up the great work.

Health care has lots of touch points where for profit fingers are in the pie that aren’t part of delivery of care. IT, auditors, claims, appeals, EHR software, coding software consultants-that’s what comes to mind right off the bat.
