if you fight the hound in the top office area and you open the office with the key card, the hound can get stuck in there and block the loot chest. If you try to fight the hound in there and you do it near a chest, you'll automatically be prompted to open the chest instead of fighting the hound. Happened to me on the high risk difficulty. Hopefully they fix issues like this
