having to explain to my new nurse every time about why i’m being so proactive is because my heart stopped a few months ago bc of a lack of medical attention and them being like “what” and i’m very nice but like yeah dude it’s all in my RECENT medical history and you’re telling me i can’t get a follow up with a specialist until december unless i go back through the ER for the sixth time this year?

I just have a template I made for apportionments that are easy read and follow. Has categories like chief complaint, symptoms, pattern/timeline, family history, things I have tried, etc. Just a 2-3 page easy summary to digest and go through that they can scan in my chart too😅
