as a child protection caseworker the number one rule is, if its not documented/recorded it never happened ... this is a psychiatrist, a dr so this is not good enough

what did you do …as a MD ???? someone licensed to Diagnose & treat! her answer was talk & validate her feelings and to go to an ER physician for treatment. what are psychiatrists doing? referring patients to ER , who send them home ? what a broken system set up to make Drs rich $$$

You can also grab a pen and paper and write it down, keep it in a confidential file to supplement the digital file. It becomes part of the patients official notes

A medical provider....nurse, physician, nurse practitioner, etc., sometimes fills out standardized flowsheets, but for THIS type of visit, the standard would be a dictated physician note. Shocking if she only used drop down boxes. 😳

It also starts with the insurance companies that only allow a provider to spend 15 minutes with one patient. Our healthcare system is lacking.

I found really hard to believe that a health professional do NOT write detailed notes, even if not in the system, at least manually on the record.

medical facilities have options to put in those little drop boxes all types of words/ diagnosis for a particular office if those boxes didn't offer certain words/ diagnosis then that office didn't care enough for them to be put in. but you also have boxes to type findings and diagnosis so that's on her she didn't care enough to put it in
