@dr.erincarter: We need to do a better job as a healthcare system in treating patients with gout to prevent long term damage and consequences. #gout #rheumatology #arthritis #medicaleducation #heartdisease
How do you know if you have tophi? If you have gout in your feet, can it move to your ankles and knees?

I was diagnosed with RA at 14 years old. I developed gout in my foot in my 20s. Didn’t know that’s what it was. I thought it was RA and I had to suffer through it. By the time it was diagnosed and treated, I had permanent damage in that foot and required surgery to fix it. Gout is so incredibly painful. Thankfully I never had it again.

A warning. After diagnosis of genetic gout (no dietary contribution), I was put on colchicine and allopurinol for occasional flares. After not having had flares for four years, I reacted to allopurinol with severe toxic epidermal necrolysis. Luckily, the GP referred me to a dermatology prof who immediately admitted me to a burns and leukemia ICU. Slathered in liquid paraffin and silicone dressings for a week, in an isolation room with heat blasting to reduce metabolic load as my skin started to repair. It's never a good thing when nurses, docs, and med students come to see you to familiarise themselves with symptoms. I now take Febuxostat daily to control the gout.

While titrating the allopurinol up, I assume you mean daily colchicine? At what dose? I have not heard of this as a primary care doctor

I don’t have a diagnosis or a doctor that listens to me and my daughter was diagnosed with psoriatic arthritis at 18 months old. I’m in pain and just want it to stop

When you say get the uric acid level down, what range are your shooting for. If it’s at the high end of normal, 347 umol/L, 6 weeks after getting allopurinol, are you happy with that. This is my second flare in my big toe.

Are meds lifelong or just temporary?

Thoughts on indomethacin for gout? I’ve seen too many AKIs from inappropriate indomethacin use when they should probably use other treatment alternatives

I went to my gp because I thought I may have kidney stones. I also have high uric acid levels and lots of joint issues. She kept asking me if I had gout. Insisting even that I should have it. But nothing was investigated or even treated. Im at a loss.

Suffering with gout for 7 years now. Don’t drink alcohol, sugary drinks, rarely eat meat or seafood. Take immunosuppressants for kidney transplant. Can’t take the preventative gout meds. Have 4-5 very painful flares year. Recently developed tophi in my fingers. Gout is debilitating and robs you of your quality of life.

I had this a couple times in my early late 30s . I have had high blood pressure since my early 20s and at the time I had gout my blood pressure wasn’t well controlled . Plus I didn’t know mackerel could cause it. I had gone fishing with my dad and we caught a ton of mackerel. I eat three times in a three week period lol I love this. I couldn’t believe I got gout at such a young age. I was put on a couple different medication’s when I was supposed to remain on, but of course I’m notorious for not doing what I’m told lol. This old guy told me drink black cherry juice and lots of water. I took his advice started drinking a little bit of black cherry juice made sure I was well hydrated and didn’t eat mackerel by the time I’ve never had a problem since and it’s been close to 20 years, but I did read somewhere along the line that uncontrolled hypertension can also lead to gout. My blood pressure is well controlled now as well.

I'm a rheumatologist in Australia and we have the same problem. It constantly baffles me how such a simple-to-fix issue causes so much suffering, while millions get poured into promoting interventions of only marginal benefit (looking at you, nintedanib)

My Dad has recurrent gout. Could the repeated episodes make him more susceptible to future attacks?

I have never had toe pain but I get recurring ankle or wrist pain that is insane. Swollen, inflamed, red. Tart cherry juice seems to clear it up, could this be gout?
