Exactly. Also to add. This woman came in for insomnia and anxiety. A PCP could handle this. But no one can manage someone who gives a history and is lying about everything

This is just a friendly suggestion as a therapist myself, I would end engagement of content centered on this trial. I think talking generally about these topics- great, but there is so much speculation, assuming, judging and just non great things going on centered on this specific case and I think as health care professionals we should really recenter the convo now.

It’s definitely definitely being pushed by the defense 😩😩 the amount of lay folks I’ve seen commenting that L’s psychiatrist was negligent and should be in jail bc she wasn’t an “expert” in postpartum psychosis and claimed she was like everyone has lost the plot????

I’m more concerned all these specialists did not recognise mania/hypomania symptoms which is clearly obvious?! She ran a marathon after giving birth, she didn’t sleep for 48 hours???

Psychosis is absolutely not difficult to diagnose in the average patients. There may be some obscure situations than it is. You aren’t going to go from zero psychosis to killing your kids overnight.

I think the larger thing here is that there needs to be better screening tools for “highly educated” or well presenting patients. Specifically for those also in the medical field that speak the language.

sure. but, people can absolutely mask symptoms of psychosis. and certain aetiologies have psychotic illnesses have more subacute onsets than others.

Doctor, are you saying you don’t believe she was experiencing a psychotic break when she killed her children? That you agree with the prosecution that she carefully planned the murders? If so, then what was her motivation? She certainly seemed to be a doting mother who loved her children.

I beg to differ. You need experience with post partum psychosis to give the treatment it requires. Its a acute illness. One of the most deadly psych illness. I think psychiatry is being a bit arrogant when you/they say all psychosis is the same. Its really not.

Idk, I was in the acute phase of postpartum psychosis for 2 months and my psychologist, physician, husband, dad, mother in law, friends all missed it. When I was lucid enough to recognize that maybe I was becoming psychotic, (I’m a psychologist) that thought would disappear moments later when the baby cried and I fell back into the belief that I was already dead and a ghost walking the earth. I’ve been working with psychosis my entire career. The other types of psychosis are easier to spot. For postpartum moms, the symptoms get dismissed as normal mom issues. It doesn’t require high specialization, but it does require a psychiatrist to do more than a 15 min video visit while never requesting records from the hospital and throwing piles of meds at the patient.

I think the lawyers on both sides fail to understand psychiatric care. It’s like they read the DSM like it’s a law book. They get hung up on diagnosis and fail to understand that that diagnosis for psychiatric disorders is not black and white!
